Download and complete the youth camp application and mail to the address seen below! *Parents must read the DOH Notice as required by the state*
The application form also includes a medical form. This should be 3 pages in all. The form is in PDF format.  Most or all should be able to open this!  If you can't, contact Joyce Terry, she can send one via Postal Service or Email!

Cost per teen for 2011: $90 - which includes living expense, food, and activities.  For those not able to pay, PLEASE contact Joyce Terry at : joyce.terry38@gmail.com
