Download and complete the youth camp application and mail to the address seen below! *Parents must read the DOH Notice as required by the state*
The application form also includes a medical form. This should be 3 pages in all. The form is in PDF format.  Most or all should be able to open this!  If you can't, contact PM at mbausman@whiteface.net, he can send one via Postal Service or Email!
